Where: Walden on Lake Houston

Time: Be at the golf club at 8:30a.m.

All athletes must bring a complete physical examination in order to participate.

Summer Hours for Registration

Monday through Thursday 8:00 AM to 3:00 PM; closed Fridays.

We will be closed June 16th - 20th and July 4th.
In order to enroll at AHS Parent and student must bring the following:

STUDENT'S OFFICIAL BIRTH CERTIFICATE

STUDENT'S SOCIAL SECURITY CARD

PROOF OF RESIDENCY - For students residing in a house, the lease, HUD Statement or current utility bill will suffice (Water, Gas, or Electric only). For apartment dwellers, either the lease with the parent and student name listed or current utility bill (Water, Gas or Electric only). All proof of residence must be dated within the last 30 days.

PARENT'S DRIVER LICENSE

IMMUNIZATION RECORD - current with the TEXAS DEPT. OF HEALTH.

SCHOOL RECORDS- LAST REPORT CARD, TRANSCRIPT, TAKS OR OTHER STANDARDIZED TEST RESULTS, PREVIOUS SCHOOLS ADDRESS/FAX NUMBER.

" form and return it to Mrs. Brown in the registrar's office (room 1007C).
