Please read this document and:

1. Print out the last 2 pages. 

Return the Medical History Form filled out.

Return the Choir Expense Form with cash/check/money order (payable to SCHS Choir Booster Club) or return it after you paid digitally.

2. Complete both Digital Forms

Student Form

• You must log into your school account to fill out the Student Form!  If it tells you that you need permission, click "Learn More." Then click on the profile picture at the right hand corner of the screen.  That will allow you to switch to your Humble ISD account and complete the quiz.

Parent Form

 

3. Take the Choir Handbook Quiz. All answers can be found in the Choir Handbook, in the document titled "Various Classroom Procedures," and in the Choir Classroom.
