Student Self-Reporting Form
Humble ISD free Covid tesing on Tuesdays 8a-4p. Must register student and a minor consent must be filled out and brought to the appointment.
Humble ISD asks people who are sick, for any reason, to remain home.
Just like every school year, families should keep children home if they are sick for any reason and staff should not come to work sick.
Humble ISD will require that students or staff with COVID-19 remain away from school for at least 10 calendar days.
If the individual is symptomatic, the days count from the date symptoms started. If the individual is asymptomatic, the days count from the date the individual was tested. After 10 calendar days have passed, individuals may return when they are 24 hours symptom-free without medication (ex. fever-free without medication).
TEA announced its requirements for the 2021-2022 school year on Thursday, August 5, and Humble ISD has ensured we are in compliance with TEA and Governor’s guidance.
The district asks all students with a positive COVID test to complete this form: COVID-19 Reporting Form for Student It only takes a few minutes to complete. We need your help in maintaining a healthy environment for your children as well as the other children in our school. In an effort to avoid students in the clinic feeling ill first thing in the morning, please use the following guidelines when determining whether or not to send your child to school. Your child is missing valuable class time by being in the clinic if he or she is ill. If you have any questions about whether or not to send your child to school, please call the clinic.
Children should be kept home from school for the following reasons:
- Coronavirus / COVID-19
- Temperature of 100 ºFahrenheit or higher – measured when no fever suppressing medication has been administered within the past 24 hours.
- Vomiting in the past 24 hours
- Diarrhea in the past 24 hours
- Persistent, hacking cough
- Continually runny nose with white, green, or yellow mucus

When calling the absence line, please let us know if your child has a fever of 100°F or more, COVID or flu like symptoms.
It is vital for your child to get plenty of rest and eat a nutritious breakfast in order to function at their best everyday.
Parents, if your children have any chronic illnesses (asthma, seizures, etc.) or life threatening allergies (food or insect stings), please contact me as soon as possible.I will need to speak to you, assess your child and provide special forms that your child's health care provider will need to complete. Please remember it is important to communicate with your child's teacher as well.
If your child needs medication at school, please contact me.ALL MEDICATIONS MUST BE BROUGHT TO THE CLINIC BY THE PARENT OR AN ADULT.NO CHILD IS ALLOWED TO BRING MEDICATION TO OR FROM SCHOOL.
LICE is a nuisance, but please do not panic. We do NOT do routine lice screening on campus. Teachers observing persistent scratching will send students to be checked. Any student with live lice will be sent home for treatment and the class will be notified.
Please keep a change of clothes in their backpacks.
We are always in need of donations such as new underwear; girls leggings; boys elastic waist bottoms & feminine products.
Nurse James wants to encourage all to stay healthy, wash your hands, don't touch your face, social distance, and stay home when appropriate to help protect others!
