    ALL STUDENTS LIVING OUTSIDE OF THE SCHOOL APPROVED BOUNDARIES MUST HAVE AN APPROVED TRANSFER TO REGISTER AT WEST LAKE MIDDLE SCHOOL. THIS INCLUDES ALL STUDENTS LIVING IN THE FLEX ZONE.


    New Student Registration 2018 - 2019

    Follow these steps to register your child:

    1. Complete the On-Line New Student Registration.
      Online enrollment available June 18th, to meet with the registrar to complete registration PLEASE CALL TO SET UP AN APPOINTMENT OR EMAILIf you do not have access to the internet, please plan to show up at least 30-45 minutes prior to your appointment time to complete the online registration. This process must be complete before you can meet with the Registrar.

    2. Bring to the campus the following required documents:

      • Current Proof of  Residence (within 30 days) in the form of either electricity bill; water bill; gas bill; lease; deed.
        NOTE: If you live in an apartment, you must present your lease, the parent's and student's name MUST be listed on the lease in order for it to be a valid proof of residency. If you are building you must provide a builder letter head stating when the closing date is, the closing must be within 30 days of enrollment. (NO exceptions) 
      • Updated Immunization (shot) records for the student. (must be signed or stamped by the doctor)
      • Last report card from previous school.
      • Official Birth Certificate
      • Social Security Card
      • Parent's drivers license 
     
    WE WILL BE UNABLE TO COMPLETE THE ENROLLMENT ON THE SAME DAY IF YOU ARE MISSING ANY NEEDED DOCUMENTATION. All of these can be obtained from the previous school ~ it is the legal right of the parent/guardian to ask for, and receive, upon withdrawal.
     
    PLEASE BE AWARE if you are NOT the student's legal guardian, you will be asked to fill out Guardianship paperwork that must be filed at the downtown Houston Courthouse.  These papers can be picked up at the Registrar's Office during normal business hours.  You cannot enroll your student until this paperwork is completed.
     
    Once your student's enrollment is completed by the Campus Registrar, set up a Home Access Center (HAC) Account. This account will allow you to monitor and view your student's academic progress, schedule, grades, test scores, and attendance. It also is used for the student's annual update registration.
     


    Returning Student Registration 2018 - 2019

    If you already have a HAC, login and complete the annual online registration, Annual HAC Update, available on July 30th. Visit our campus homepage for more information about turning in the verification of residence. 

    ALL STUDENTS
     (new and returning) will be required to complete registration forms online.  Proof of residency will be required for all new students and returning students. The registration packets can be downloaded from the district's or campus' website. Forms are available in both English and Spanish.  The forms include:

    1. Student enrollment (new students only)
    2. Compulsory Attendance Form
    3. TEA Ethnicity and Race Data Questionnaire
    4. Student Residency Questionnaire
    5. Parent Signature Form
    6. Migrant Student Survey Physician
    7. Emergency Card Student
    8. Health Inventory
    9. TB Form
