     Over the summer physicals can be turned in at the Humble ISD Main Athletic Office from 10AM to 2PM on the dates below.

    June 13 Physical Drop off at Athletics
    June 20 Physical Drop off at Athletics
    June 27 Physical Drop off at Athletics 
    July 3   Physical Drop off at Athletics
    July 11 Physical Drop off at Athletics
    July 18 Physical Drop off at Athletics
    July 25 Physical Drop off at Athletics
     
     
    2018-2019 Online Athletic Paperwork

     All Athletes must read and sign all of this year’s paperwork online.  This will also include your emergency card for traveling.  Please fill these forms as soon as possible with a guardian.  They can be completed on any computer or smartphone.

    • Click Start Online Forms
    • Click “I don’t want an account, go to forms”
    • Click “I don’t want an account, go to forms” again
    • You will need to complete all 5 (6 if applicable) of the forms
    • Type in your name, School ID number, and school.
    • Read and complete the form with Guardian signatures, and repeat for the other forms.
    • Complete all 5 or 6 forms as applicable
