2020-2021 Online Athletic Paperwork

All Athletes must read and sign all of this year’s paperwork online. This will also include your emergency card for traveling. Please fill these forms as soon as possible with a guardian. They can be completed on any computer or smartphone.

• Click the top tab “Electronic Participation Forms”

• You will need to complete all 6 of the forms

• Type in your name, School ID number, and school.

• Read and complete the form with Guardian signatures, and repeat for the other forms.
