    Hey, it looks like you don't have a PDF viewer plugged into your browser. If that's so, then good for you. Big fat plugins (like literately any Adobe product) can really bog down the browser. Download the PDF file here to edit it in a stand-alone PDF program. Any program that supports form filling will work. There's two buttons with JavaScript attached to them but neither are necessary, they just make it a little easier (and introduce a lot of potential security and privacy risks).

    It's also possible that you're seeing this message because I lost the PDF file that was supposed to be here. If that's the case, then bad for me. Also the above link won't work.
