• Required Immunizations

    All students must be immunized in accordance with state law before entering Texas schools. Vaccines are available from doctors' offices, the Harris County Public Health and Environmental Services Department, 281-446-4222, or the Houston Department of Health and Human Services, 713-794-9320.
     
    Information regarding immunizations requirements, in both English and Spanish, can be accessed here. 
     
    When turning in records, including electronic (My Chart) records, Records must be signed or stamped by the physician or his/her designee or public health personnel.

    Affidavits
    Affidavits regarding varicella are available. Please print the varicella proof form

    Exemptions
    For children who need medical exemptions, a written statement by the physician should be submitted to the school annually. Parents/guardians choosing the exemption for reasons of conscience, including a religious belief, should visit www.ImmunizeTexas.com for instructions.

     
    3 Year Old
    Polio - 3 doses
    DTaP - 4 doses
    MMR - 1 dose - must be given after 1st birthday
    Hepatitis B - 3 doses
    Varicella - 1 dose - after the 1st birthday

    Haemophilus Influenzae type B (Hib) - 3 doses  1
    Pneumococcal - Complete series or 1 dose after age 2  2
    Hepatitis A - 1 doses  after 1st birthday

    1   A complete series is 2 doses plus a booster  with 1 after 1st birthday.  If 1st dose is given after 1st BD only one additional dose is required.  If 1st dose is given after 15 months of age, only one dose is required

     2   A completed series - 4 doses.  3 doses with one after the 1st birthday, or 2 doses with both after 1st birthday, or one dose after 2nd birthday

    DTP/DT/DTaP- 3 doses primary series and 1 Tdap/TD within 5 years
    Hepatitis A  2 doses
    Hepatitis B - 3 doses
    Varicella - 2 dose - 1st dose after 1st birthday2     
    MMR 2 doses - Student vaccinated prior to 2009 with 2 doses of measles and one dose of rubella and mumps meets requirement 
    Meningicocal (MCV4) - 1 dose

    Polio - 4 doses with 1 after 4th birthday 1

    Tdap/DT - booster 2
    MMR 2 doses  Student vaccinated prior to 2009 with 2 doses of measles and 1 dose rubella and mumps satisfy this requirement
    Hepatitis B - 3 doses
    Varicella  - 2 doses - first must be given after 1st birthday3
    Meningococal – 1 dose

    Students in Classes on College Campus
    All students attending classes at Quest/Early College or any other college campus (including Community Colleges) must have a Meningococcal immunization within the past 5 years. The immunization must be administered more than 10 days prior to attendance.

    RECOMMENDATIONS
    • Flu Vaccine yearly
    • HPV for boys and girls over the age of 9 is strongly recommended. 
    • Meningitis B vaccination ages 16-25
