- How do I get a slow pass for my child?
- If it is needed for less than a week, please send a note or email us.
- If it is needed for longer than a week, we need a physicians note with the type of injury or surgery and a specified end date.
- All elevators here at KHS are open for use without a key.
- My child needs as needed medication, what is the process?
- Parent need to complete the medication form found on our website and bring in a small (50 count or less) bottle of medication.
- If it is a daily medication, the medication form must be filled out by the parent and the physician, the parent will bring in the medication bottle.
- When do I need to have a physician sign medical forms?
- Any prescription medication.
- Any medication given four or more times a month.
- Any medication that the dose is greater that what is on the bottle.
- All Inhaler, Epi Pen, Seizure medications.
- Any G-Tube feedings.
- How do I sign my child out when ill?
- If your child calls or texts you, please have them come to the clinic, we can assess them and they can then call you to pick them up or to bring medication.
- If your child doesn't come to the clinic, please make arragements through the attendance or house office.
- I am concerned that my child has a vision or hearing issue, can they be screened in the clinic?
- Yes, please contact us and we will make arrangements to screen your child.
- All new students to the district will be screen for hearing, vision and Antes.
- My child just got new vaccines, should I notify the nurse?
- Yes, please send any new vaccination your child recieved and we will update our records.
