New Immunization Requirements for 2009-2010 School Year

All incoming 7th Graders will need a Tdap, Meningitis & a 2nd Varicella. (Chicken Pox)

ALL STUDENTS (new and returning) will be required to complete registration forms in writing.  The forms will be returned to the school at the above times for returning students.  New to district students can call 281-641-3807 after August to schedule an appointment for enrollment.  Proof of residency will be required for all new students and returning students. The registration packets can be downloaded from the district's or campus' website. Forms are available in both English and Spanish.  The forms include:

(3) Student residency questionnaire

(4) Migrant student survey

(5) TB Questionnaire

(6) Physician's Request Dietary Modification Form

(7) Compulsory Attendance Form 

(8) Emergency Card

(9) Parent Signature Form

(10) TEA Ethnicity and Race Questionnaire

 

Proof of residency includes the following:  Homeowners:  current utility bill(water, gas, electric) or new home closing papers; for Renters:  residential lease agreement with signatures; for an apartment: a copy of the current lease showing the parent and student names will be required.

 

Home Access Center (HAC) allows parents to access their student's attendance and grading information from any Internet-connected computer.  Parents will be able to see their student's information in real time, as soon as the information is posted by the teachers.  Access information can be found at:  https://eshac.humble.k12.tx.us/homeaccess/
