Ms. Pack's goal is for each CMS student to be safe and well during the school day and to receive prompt attention if medical problems arise. Students who are feeling ill during class should sign up between classes. Those who have more serious problems, such as bleeding, vomiting, high fever, convulsions, and migraines will be taken care of immediately. Students may not carry any type of medication without permission from the nurse. All medications must be taken to the clinic by an adult. Prescription drugs or over the counter medication will require a doctor's note. If these guidelines are followed, all CMS students will receive the health care they need.


7th Grade Immunization Requirements

 

7th Grade

Polio - 4 doses with one after the 4th birthday 1

DTaP - 3 doses with one after 4th birthday

Tdap/DT – booster 2

Measles - 2 doses with first after the 1st birthday 3

Rubella - 1 dose after the 1st birthday

Mumps - 1 dose after the 1st birthday

Hepatitis B - 3 doses

Varicella - 2 dose 4, 5

Meningococcal – 1 dose

 

1- 3 doses meet the requirement if 3rd given after 4th birthday

2- Only required if more than 5 yrs since last dose of tetanus containing vaccine.

3- May be a measles containing vaccine such as MMR

4- Previous illness maybe documented w/written statement.

5- 2 doses with first after the 1st birthday

 

 

 

Immunization Clinics will be at the following dates and locations.

Aug 13,              Tuesday                    7:30 am – 12:00 pm    Atascocita Middle

Aug 14               Wednesday               2 -7 pm                       Timberwood Middle

Aug 22               Thursday                   2-6 pm                        Kingwood Middle

Aug 23               Friday                       10am - 2pm                 Ross Sterling Middle

 

 

   Vaccines will be $10/immunization.  Cash or check made payable to Texas Vaccine Institute is accepted.  Parent/guardian must be present and copy of immunization records must be provided.  Medicaid Texas Children’s, Blue Cross Blue Shield-TRS and Aetna accepted with insurance card verification.

 

Immunization records need to be returned to the school nurse so that your child’s school health record can be updated as soon as possible. If immunization records are not provided to the nurse by, August 15th, your child will notreceive the registration packet
