Required Immunizations
All students must be immunized in accordance with state law before entering Texas schools. Vaccines are available from doctors' offices, the Harris County Public Health and Environmental Services Department, 281-446-4222, or the Houston Department of Health and Human Services, 713-794-9320.

Affidavits
Affidavits regarding varicella are available at www.humbleisd.net. Go to Helpful Links and Information to print the varicella proof form.

Exemptions

For children who need medical exemptions, a written statement by the physician should be submitted to the school annually. Parents/guardians choosing the exemption for reasons of conscience, including a religious belief, should visit www.ImmunizeTexas.com for instructions.

 

3 Year Old
Polio - 3 doses
DTaP - 4 doses
MMR - 1 dose - must be after 1st birthday
Hepatitis B -3 doses
Hepatitis A - 2 doses - first dose after the first birthday
                                 second dose 6 months after 1st
Varicella - 1 dose - after the first birthday
Pneumococcal - Complete series with one dose after age one or one dose after age two
Haemophilus Influenzae type b (HIB) - Completed series or one after 15 months.

Varicella  - 2 dose First dose after first BD 5,6
Meningococal – 1 dose

No Meningitis required
One Varicella required.
